Billing and Payment Policy

Patients without insurance: If you are uninsured and self-pay at the time of your appointment, you will receive a 15% same-day discount if you do not have an outstanding account balance with us. If you have an outstanding account balance with us, or you are not able to pay at the time of your appointment, you will receive a bill for the full cost of the services provided to you.
Patients with insurance: While we are in network with a number of plans, patients are responsible for checking with their insurance provider to see if we are in the network for the patient's specific plan.
Co-payments: You will be asked to pay all co-payments at the time of service.
Other charges: If your health insurance company does not cover 100% of the services rendered, you are responsible for any co-insurance, deductibles or non-covered services that are not paid by your insurance. You will receive a statement from our office indicating the amount your insurance has paid and the amount you owe. Any balance remaining is due from you within thirty (30) days of your receipt of the statement.
First statement: Regardless of your insurance status, when we determine that you owe a balance, we will mail a statement to the mailing address provided to us by you. If that address changes, you are responsible for letting us know in writing. All statements are also available on our secure patient portal. Payment is due from you to us within thirty (30) days of your receipt of the statement.
30-days past due: In the event you fail to make the payment within 30 days, we will mail you a second statement.
60-days past due: A phone call will be made to the guarantor on the account. If payment is not made within 5 days the account will be turned over to Centron Collections Agency. 
Payment plan: We require patient with balances over $200 to enter into a payment plan with a credit card or checking account on file and automated payments scheduled.  10% of the total balance or $50, whichever amount is greater, is due the first day of the payment plan and each month following.
